Overview.

Art directed the full rebrand of Heartland Health Centers to Tapestry 360 Health, a Chicago-based community health organization dedicated to providing quality, accessible, and affordable healthcare. Developed the new visual identity anchored by a multicolor 360-degree arc logo mark and the tagline 'Together For Better Health' — reflecting the organization's holistic, patient-centered approach to care. Built a woven geometric pattern system and a warm, inclusive color palette of orange, teal, and navy, paired with diverse community photography that represented the patients and families the organization serves. Delivered the complete launch campaign across stationery suite, direct mail postcards, Instagram social content announcing the name change, digital web banners, CTA train interior ads, and CTA bus shelter placements throughout Chicago neighborhoods.
